Drivers Responsibility

It is the client's responsibility to bring in all the necessary paperwork for their assessment. Failure to do so may lead to a fail or the rescheduling of an appointment

Documents To Bring

Every client must have an in-date form of ID e.g. a Driving license or Passport

You must also bring a D4 form, this can be obtained from any post office or printed online from the link below

If you have had a hospital admission in the last year, please bring a copy of the discharge summary

If you have been seen in an outpatient clinic within the last 5 years, please bring a copy of the clinic letter

If you wear contact lenses or glasses to drive, please bring in a copy of your prescription strength from the opticians

If you are on medication for high blood pressure you must bring evidence of 3 previous blood pressure readings

If you wear glasses or contact lenses to drive, please bring in a copy of your prescription strength from the opticians

If you fail only on the vision assessment, the rest of the form can be filled in and the vision assessment can be completed by your local opticians

If you are on medication for high blood pressure, must bring evidence of 3 previous readings

If you take your blood pressure medications every morning you must take it at least 45 minutes before your medical

If you fail only on blood pressure you will be eligible for one free retest

If you have a history of ischemic heart disease i.e. previous angina or heart attack, you must have a stress echocardiogram every 3 years. Please bring evidence of this to your assessment

Please bring any evidence of outpatient appointments or scan results within the last 5 years

Medication

If you suffer from type 1 diabetes you must bring evidence of 6 continuous weeks' worth of blood sugar readings stored on a monitor

Please bring in a list of any regular medication you are prescribed
